We Believe Hospice Care Should be Compassionate & Individualized

Legacy Hospice is dedicated to providing compassionate, individualized, and dignified care to those we serve. We believe that hospice should be patient driven and family centered as no two people are the same. A team approach to end-of-life care is pivotal to ensure patients and their loved ones have all physical, emotional, and spiritual needs met.

Comprehensive Hospice Care Services

In remembrance of our own loved ones lost along the way, our team has built a foundation of love that is reflected in our care for others. We will walk with our patients on their end-of-life journey, guiding and supporting them each step of the way. We are honored to be a part of their legacy.

Nursing

Registered nurses will develop a plan of care and perform routine visits, symptom managment and medication management.

Nurse Aides

A certified nursing aide (CNA) performs personal care and assists the patient with ADLs (activities of daily living)

Spiritual Counseling & Bereavement

Spiritual care is a necessary piece of end-of-life care. Bereavement counseling helps patients and their loved ones who are dealing with grief and loss. 

Nutritional Counseling

Dieticians collaborate with the patients, families and caregivers to address unique dietary needs and provide hospice specific nutritional counseling.

Social Work

Social worker’s biggest priorities include advocating for end of life wishes, providing emotional support, discussing POA/MPOA paperwork and providing resources.

PT/OT/Speech Therapy

We contract with a home health agency that provides physical therapy, occupational therapy and speech to our patients when the need arises.  

Common Hospice Questions

Hospice is a type of care that ensure patients are treated for their symptoms and not the cure of a terminal illness. The focus is quality of life, comfort and support physically, emotionally, and spiritually. We care for people wherever they call home. 

Hospice is for people who have a life-limiting illness with a life expectancy of 6 months or less, if the disease follows its normal course. Hospice should be considered sooner than later to receive the maximum benefits and full scope of hospice services available and to help maintain a quality of life of those we serve.

Hospice is a 100% Medicare covered benefit and is also covered by many private insurance providers. The hospice benefit covers medications, supplies, and equipment related to the hospice diagnosis.
